Both parents will need to fill out this parental consent form if petitioning for a minor’s name to be changed.

A petition for name change, civil cover sheet, and notice of publication for name change all need to be presented to a court in order to file a name change, regardless of age. Exceptions to the publication process exist for victims of domestic violence, especially in cases involving a restraining order.

If changing your name as a result of marriage, these forms are NOT needed. A marriage certificate can be presented to your local social security office and driver’s license office to make this change.
